Baptist Health is looking for a HEDIS Pharmacy Outreach Coordinator to join our team. This is a remote position that requires residency in KY or IN

Under direction from the Director of Clinical Outcomes for Value Based Care, responsible for the performance on the HEDIS pharmacy measures for our value-based contracts, both for BHMG and the Clinically Integrated Network (CIN). Subject matter expert on HEDIS Pharmacy measures, including CMS, and NAHQ-HEDIS specifications. Researches and maintains database of best practices for success in the value- based program. Proactively monitors HEDIS Pharmacy performance throughout the year. Organizes and presents accurate reporting to instill confidence in our program. Maintains a positive working relationship and uses effective communication with patients, payer partners, Population Health team, support staff, physician and hospital leadership. Assists with educational effort regarding the HEDIS Pharmacy measures. Works closely with Primary Care Clinical Outcomes Coordinators. Proactively identifies and reports any special needs and potential problems to leadership to ensure the program's success. Demonstrates necessary interpersonal skills and maintains a positive and professional atmosphere in the department. Follows system and departmental policies and procedures.

Minimum Requirements

  • LPN Diploma licensed in the state of Kentucky with at least three years' experience.

  • Patient education, patient outreach and HEDIS experience preferred.

  • Requires proven ability to work closely and collaboratively with healthcare professionals and leadership; excel in both written and verbal communications; proficient with Microsoft Office products; knowledge of medications, including pronunciation and indications; and strong customer service skills.
